Family raised concerns about NHS mental health care before William Chapman was mauled to death by tiger, inquest hears
William Chapman, the former child actor killed by a tiger at Yorkshire Wildlife Park, whose family said he had suffered 'significant mental illness over a number of years' and had been prescribed 'strong medication.'
William Chapman's family had expressed "a number of concerns" about his contact with mental health services before the 24-year-old climbed two fences and entered the tiger enclosure at Yorkshire Wildlife Park on October 10. The inquest opening at Doncaster Coroner's Court heard Chapman had a diagnosis of schizophrenia and had suffered previous bouts of psychosis, yet was classed as "highly functioning and independent with no care requirements" by Doncaster Mental Health Services.
Rotherham, Doncaster and South Humber NHS Foundation Trust has launched a Patient Safety Incident Investigation into his contact with services.
Chapman died of "multiple traumatic injuries and animal interaction" after the 200kg Siberian tiger Altai attacked him at about 1.30pm.
The full inquest is set for March 18 next year; Chapman's family called him 'a bright, warm and engaging young man.'
