NHS ambulance staff are probed over claims they accessed records of a man mauled to death by a tiger

Yorkshire Ambulance Service NHS Trust says two of its staff may have viewed or tried to view William Chapman's medical information 'without a legitimate reason'. Chapman, 24, died after climbing two fences into the tiger enclosure at Yorkshire Wildlife Park near Doncaster on 10 October. The trust's own audits flagged the potential breaches. It has reported them to the Information Commissioner's Office and told his family.

Chapman had schizophrenia, and his family raised 'a number of concerns' about his mental health care.

A separate Doncaster mental health trust has launched its own investigation into how it dealt with him.

The park reopened on Friday, and Altai, the 200kg tiger, is back on show to visitors.
