NHS ambulance staff investigated for allegedly snooping on medical records of man mauled by tiger

Yorkshire Ambulance Service NHS Trust is investigating two members of staff suspected of accessing — or attempting to access — William Chapman's medical records without authorisation following his death at Yorkshire Wildlife Park. The trust has referred the potential data breach to the Information Commissioner's Office and informed Chapman's family, who had already voiced concerns about the mental health care he received before the attack. The violations were uncovered through the trust's own proactive auditing. Chapman, 24, died on Saturday after climbing two fences and entering the enclosure of Altai, a 200kg Siberian tiger, in Doncaster.
An inquest heard Chapman had schizophrenia and previous bouts of psychosis, but was 'highly functioning and independent with no care requirements'.
His family has raised 'a number of concerns' about his dealings with mental health services in the period before his death, prompting a separate NHS investigation.
Yorkshire Wildlife Park reopened on Friday, with Altai — who will not be euthanised — pictured eating meat in his enclosure.
