NHS trust is prosecuted over sepsis deaths of two girls after one was sent home by a 'very junior doctor'

Zadie Ajetunmobi, three, went more than seven hours without intravenous antibiotics at Broomfield Hospital in Essex and died within 10 hours in November 2022. Selina Samarina, two, who had Down's syndrome, died two days after a doctor discharged her in April 2024. The Care Quality Commission has brought three charges against Mid and South Essex NHS Foundation Trust. It is due before Chelmsford Magistrates' Court in December.

Zadie's family were told the trust's sepsis guidance had not been updated since 2017.

No senior doctor was available for two-and-a-half hours after Selina triggered the sepsis protocol.

Parents Theo and Rhiannon: 'This is a tragedy from which we will never recover.'
