Ohio mayor blames employee error for Toledo chemical leak which left 60 people in hospital, and lifts evacuation orders

Residents forced from their homes in Toledo, Ohio, have been told they can return after the last evacuation orders were lifted. The city's mayor pinned the leak on a single worker's mistake, but admitted key questions about what went wrong still have no answers. An investigation into the hazmat release is continuing.
Some 60 people needed hospital treatment after the leak.
The mayor said the inquiry is not over, and questions remain unanswered.
