Two NHS ambulance staff are investigated over claims they accessed the medical records of a man mauled to death by a tiger
William Chapman, the former child actor killed by a tiger at Yorkshire Wildlife Park, whose family said he had suffered 'significant mental illness over a number of years' and had been prescribed 'strong medication.'
William Chapman, 24, had his medical records accessed or targeted on two occasions "without a legitimate reason", Yorkshire Ambulance Service NHS Trust has confirmed. The trust's internal audits identified the two staff members following crews' response to the 10 October emergency at Yorkshire Wildlife Park in Doncaster. The Information Commissioner's Office has been notified, and Chapman's family, who are still grieving, have been told about the investigation.
Chapman had schizophrenia, and the Doncaster inquest has been adjourned until March 18.
Rotherham, Doncaster and South Humber NHS Trust is separately reviewing its mental health contact with him.
Altai, the 200kg Amur tiger that killed Chapman, was back on public display when the park reopened on Friday.
