Two NHS ambulance staff investigated over William Chapman's medical records after tiger mauling death
William Chapman, the former child actor killed by a tiger at Yorkshire Wildlife Park, whose family said he had suffered 'significant mental illness over a number of years' and had been prescribed 'strong medication.'
Yorkshire Ambulance Service NHS Trust is investigating two members of staff who gained or attempted to gain unauthorised access to William Chapman's medical records following his death at Yorkshire Wildlife Park. Chapman, 24, was killed by Altai, a 200kg Siberian tiger, after scaling two fences into the enclosure on 10 October. The trust has reported the potential breaches to the Information Commissioner's Office and notified Chapman's family.
The trust says its own proactive audits uncovered the issue, rather than a tip-off from an external whistleblower.
At Chapman's inquest it emerged he had schizophrenia and experienced psychosis, and his family has raised questions about how mental health services engaged with him in the period before he died.
Chapman filmed himself in his final moments, asking park-goers to take photographs of him with the tiger before entering the enclosure wearing headphones.
