Yorkshire Ambulance Service probes two staff members over William Chapman's medical records accessed 'without legitimate reason'
William Chapman, the former child actor killed by a tiger at Yorkshire Wildlife Park, whose family said he had suffered 'significant mental illness over a number of years' and had been prescribed 'strong medication.'
Yorkshire Ambulance Service NHS Trust is investigating two employees who looked at or tried to look at the medical records of William Chapman — the 24-year-old killed by a Siberian tiger at Yorkshire Wildlife Park — without any legitimate reason to do so. The trust uncovered the potential breach through its own proactive audits and has since referred the matter to the Information Commissioner's Office. Chapman's family has been notified about the investigation.
Chapman died on 10 October after scaling two fences and entering the enclosure of Altai, a 200kg Amur tiger, at the Doncaster wildlife park.
An inquest heard Chapman had schizophrenia and 'multiple traumatic injuries' were listed as provisional cause of death, with a full hearing adjourned until March.
The trust said it has 'commenced an investigation to establish the full circumstances surrounding these potential breaches and the actions of these individuals.'
